SOP: Fall Protocol
Follow this protocol immediately after any resident fall to ensure safety, complete documentation, and timely family notification.
Immediate actions (within 0–5 minutes)
- Do not move the resident until assessed for injury unless in danger.
- Call for help using the pendant or radio; initiate Code Fall if needed.
- Check airway, breathing, circulation; apply first aid as trained.
Assessment and notification (within 15 minutes)
- Nurse assessment and vitals; neuro checks if head impact suspected.
- Notify provider per standing orders; notify Responsible Party.
- Complete incident note in EHR and fall event in eMAR if medication-related.
Post-fall huddle (within 24 hours)
- Review root causes; update care plan and fall risk score.
- Implement interventions (hip protectors, non-slip socks, room setup).
Checklist
- Scene secured and resident reassured
- Injury assessed and documented
- Family and provider notified
- Incident report submitted
- Huddle completed and plan updated
