SOP: Fall Protocol

Follow this protocol immediately after any resident fall to ensure safety, complete documentation, and timely family notification.

Immediate actions (within 0–5 minutes)

  • Do not move the resident until assessed for injury unless in danger.
  • Call for help using the pendant or radio; initiate Code Fall if needed.
  • Check airway, breathing, circulation; apply first aid as trained.

Assessment and notification (within 15 minutes)

  • Nurse assessment and vitals; neuro checks if head impact suspected.
  • Notify provider per standing orders; notify Responsible Party.
  • Complete incident note in EHR and fall event in eMAR if medication-related.

Post-fall huddle (within 24 hours)

  • Review root causes; update care plan and fall risk score.
  • Implement interventions (hip protectors, non-slip socks, room setup).

Checklist

  • Scene secured and resident reassured
  • Injury assessed and documented
  • Family and provider notified
  • Incident report submitted
  • Huddle completed and plan updated
